​UNUM Policy Information

Upon being hired, all Active Members are enrolled in a Term Life Group Insurance Policy through UNUM Life Insurance Company. The policy is for $50,000 and the proceeds of this benefit will be paid to your beneficiary of record after the Fund office receives proof of your death. Please make sure that your beneficiary information is updated and current. Changes can be made at any time by contacting the Welfare Fund. This benefit is discontinued upon the member’s retirement or termination. Members have the option to port this benefit upon retirement or termination.

As part of this policy, members have access to travel insurance at no cost to them.

Employee Assistance Programs (EAP)

Employee Assistance Programs can improve well being and provide a variety of health and productivity services. Welfare Fund members have access to UNUM’s EAP program at no cost to them. It is a resource for employees to cope with depression, mental illness or substance abuse. Our EAP also assists members with concerns that impact life at home and at work. Services include information regarding child care and elder care, relationship issues and other stress related life challenges.

Statement of Grandfathered Status

The New Rochelle FUSE Welfare Fund Welfare Fund believes this Plan is a “grandfathered health plan” under the Patient Protection and Affordable Care Act (the Affordable Care Act). As permitted by the Affordable Care Act, a grandfathered health plan can preserve certain basic health coverage that was already in effect when that law was enacted. Being a grandfathered health plan means that your Plan may not include certain consumer protections of the Affordable Care Act that apply to other plans, for example, the requirement for the provision of preventive health services without any cost sharing. However, grandfathered health plans must comply with certain other consumer protections in the Affordable Care Act, for example, the elimination of lifetime limits on benefits.
